  THREE FORMS must be completed for application for employment. All three are available on this website.

    Click to download an Reference Release Form.  
    Click to download an Iowa Health Care Facility (135C) Record Check. (This is an Iowa Criminal History/Dependent Adult Abuse Check required by the State of Iowa).  
  The application process will be "delayed" until ALL THREE forms have been received by the Human Resources Department.  
       
  After downloading the PDF files:  
    1) Open each file (requires Adobe Acrobat Reader - a free download from Adobe.com).
2) In the PDF toolbar immediately above the document, click on "Actual Size" or "100%."
3) Print the document.
4) Complete the Application Form.
5) Complete the Record Check Form (only the 8 blank fields designated by an "X").
