Bethany Life Communities' Transitional Care Unit bridges the gap between hospital and home for patients with complex or multiple health concerns due to illness, injury or surgery. We focus on individual goals to help you achieve your highest level of independence.
We are a regional center of excellence in Transitional Care, currently serving patients from communities in Story, Boone, Hamilton, Hardin, Webster, Marshall and Polk counties.
